Pragmatic Free Trail Meta is an open data platform that enables research into pragmatic trials. It gathers and distributes clean trial data, ratings, and evaluations using PRECIS-2. This permits a variety of meta-epidemiological analyses that compare treatment effect estimates across trials of various levels of pragmatism.

Background

Pragmatic studies are increasingly acknowledged as providing evidence from the real world for clinical decision-making. The term "pragmatic" however, is used inconsistently and its definition and assessment require further clarification. The most pragmatic trials should not blind participants or clinicians. This could lead to an overestimation of treatment effects. The pragmatic trials also include patients from various healthcare settings to ensure that their results can be applied to the real world.

Additionally, clinical trials should concentrate on outcomes that are important to patients, such as the quality of life and functional recovery. This is especially important for trials involving invasive procedures or those with potential serious adverse events. Despite these requirements however, a large number of RCTs with features that challenge the notion of pragmatism were incorrectly labeled pragmatic and published in journals of all types. This can result in misleading claims of pragmaticity and the usage of the term must be standardized. The creation of a PRECIS-2 tool that can provide a standardized objective assessment of pragmatic features is a good start.

Methods

In a practical study the aim is to inform clinical or policy decisions by showing how an intervention can be integrated into routine care in real-world situations. This is different from explanatory trials that test hypotheses about the cause-effect relationship in idealised settings. In this way, pragmatic trials can have less internal validity than explanation studies and be more susceptible to biases in their design, analysis, and conduct. Despite their limitations, pragmatic research can provide valuable data for making decisions within the context of healthcare.

The PRECIS-2 tool evaluates an RCT on 9 domains, ranging from 1 to 5 (very pragmatic). In this study the domains of recruitment, organisation as well as flexibility in delivery flexible adherence and follow-up were awarded high scores. However, the primary outcome and the method for missing data scored below the pragmatic limit. This suggests that it is possible to design a trial that has good pragmatic features without harming the quality of the results.

The original PRECIS tool3 was an adapted version of the PRECIS tool3 that was based on the same scale and domains. Koppenaal et. al10 devised an adaptation of this assessment, dubbed the Pragmascope, that was easier to use for systematic reviews. They discovered that pragmatic reviews scored higher in most domains, but scored lower in the primary analysis domain.

The difference in the primary analysis domains could be due to the way in which most pragmatic trials approach data. Some explanatory trials, however don't. The overall score was lower for pragmatic systematic reviews when the domains of organisation, flexible delivery and follow-up were combined.

It is important to note that the term "pragmatic trial" does not necessarily mean a low-quality trial, and there is an increasing rate of clinical trials (as defined by MEDLINE search, but it is neither specific or sensitive) that use the term 'pragmatic' in their title or abstract. The use of these words in abstracts and titles could indicate a greater understanding of the importance of pragmatism but it is unclear whether this is reflected in the content of the articles.

Conclusions

In recent years, pragmatic trials are gaining popularity in research as the importance of real-world evidence is becoming increasingly acknowledged. They are randomized clinical trials which compare real-world treatment options instead of experimental treatments under development. They have patients which are more closely resembling the ones who are treated in routine care, they employ comparators that are used in routine practice (e.g., existing medications) and depend on the self-reporting of participants about outcomes. This method could help overcome limitations of observational studies that are prone to limitations of relying on volunteers, and the limited accessibility and coding flexibility in national registries.

Pragmatic trials also have advantages, such as the ability to use existing data sources and a greater probability of detecting meaningful differences than traditional trials. However, pragmatic tests may be prone to limitations that undermine their effectiveness and generalizability. The participation rates in certain trials may be lower than anticipated due to the health-promoting effect, financial incentives or competition from other research studies. The necessity to recruit people quickly reduces the size of the sample and the impact of many practical trials. Certain pragmatic trials lack controls to ensure that the observed variations aren't due to biases in the trial.

The authors of the Pragmatic Free Trial Meta identified RCTs that were published between 2022 and 2022 that self-described themselves as pragmatic. They assessed pragmatism by using the PRECIS-2 tool, which includes the domains eligibility criteria and recruitment criteria, as well as flexibility in adherence to interventions and follow-up. They found that 14 trials scored highly pragmatic or pragmatic (i.e. scoring 5 or more) in at least one of these domains.

Trials that have a high pragmatism score tend to have more expansive eligibility criteria than traditional RCTs that have specific criteria that are unlikely to be found in clinical practice, and they include populations from a wide variety of hospitals.
